Doctor Permits are only valid in specified doctors bays. The permit is not valid in permit holder, pay and display, or loading bays, on yellow line waiting restrictions or in taxi ranks.

Doctor Permits are available to the following Doctors Surgery’s that have been authorised by West Sussex County Council –

• Langley House Surgery, West Street
• Consultation Rooms, West Street
• Parklands Surgery, Parkland Road

To use this form to apply for a permit, you will need to provide a copy of the following:

• A dated letter on headed paper confirming you are the correct surgery applying for the doctors permits.

Please be aware that without the correct documentation your application will be rejected and you may need to reapply.
